Often, yes. Most stalls trace to an unattested CAQH profile, an unanswered development request, or an application sitting in a queue nobody has followed up on. Diagnosing which of the three is the first step.
Do we need a formal compliance program if we are a small practice?
The elements scale, but the expectation does not disappear with size. A proportionate program — written standards, a named responsible person, training, screening, periodic audit and a response process — is achievable for a small practice and materially changes your posture if you are ever audited.
Can you handle a practice with multiple locations and tax IDs?
