Policies & Procedures
A policy manual is only useful if it matches the services you actually deliver and the standards your surveyor actually applies. We write custom, service-specific manuals — not templates with your logo dropped on the cover — and we keep them current as rules change.
What is included
- Administrative and governance policies, including governing body and administrator responsibilities
- Clinical and service-delivery policies specific to your disciplines and settings
- Personnel policies, job descriptions, competency requirements and supervision structures
- Patient intake packets, consents, notices and rights documentation
- Infection prevention and control policies
- Emergency preparedness policies coordinated with your all-hazards plan
- HIPAA privacy and security policies, breach notification and business associate management
- Quality assessment and performance improvement structure
- Delivery as an editable, version-controlled electronic manual stored in an encrypted cloud account
How the engagement runs
- Service inventory — we document exactly what you deliver, where, by whom and under whose supervision, because that determines which standards apply.
- Standards mapping — state licensure rules, Medicare conditions of participation or coverage, accreditor standards and payer contract obligations.
- Drafting — policies written to your operations, with procedures a new employee could follow.
- Review cycle — your leadership reviews and adopts; we revise until it reflects practice rather than aspiration.
- Maintenance — updates as rules change, with a change log that shows a surveyor when and why each revision occurred.
Who this is for
Any licensed or certified provider, but particularly new agencies, organizations that have outgrown a purchased template manual, and providers who have been cited for policies that did not match observed practice.
Frequently asked questions
Why not just buy a template manual?
Because the most common survey finding is not a missing policy — it is a policy that does not match what the organization actually does. Template manuals describe a generic organization, and a surveyor comparing the manual to your practice will find the gap. A manual that overstates your controls is worse than no manual.
How often do policies need to be reviewed?
At minimum annually, and immediately upon any regulatory change affecting your service lines. Surveyors look for evidence of periodic review, not just a current document, so the review record matters as much as the content.
Do you provide the forms as well as the policies?
Yes. Procedures that reference a form no one can produce are a common deficiency, so intake packets, consents, competency checklists and logs are delivered with the manual.
